Our mission is to improve the outcomes and reduce the cost of support for individuals. This is achieved by providing informed care though improved clinical insight, training and support for the psychosocial and behavioural factors that impact on the person’s care needs.
We provide a full suite of evidenced based interventions. From detailed assessments to the development of personalised care and support plans; especially for people with challenging behaviours, and that are on 3:1 levels of support, and/or on psychotropic medications.
The assessment and intervention phase is delivered at our state-of-the-art assessment centre. This is a Smart House and living quarters for the person, which also provides access to Day Opportunity and Training Centre.
During the care plan development and implementation phase our team works closely with the individual and their circle of support to understand their unique needs, preferences, and to develop realistic lifelong goals. We believe in building strong partnerships and empowering individuals to actively participate in their care journey and we work in collaboration with the local healthcare system, local authorities and social services to achieve this.
We also identify any specific training needs for implementing the Lifelong Plan and provide training if needed.
Once the Lifelong Care Plan is in place, we provide outreach support to ensure a seamless transition between services and continuous support as the individual’s requirements evolve.
